To book a consult, please visit our website at www.evateleconsult.com and click “Book Now”. Kindly select a service, pick the date and time slot that works best for you, choose your preferred doctor and indicate your health concern.
Next, you will be redirected to the login page. If you do not have an existing account, please select “Create Account”. Kindly fill out the patient’s details such as the name, gender, mobile number and a working email address. If you have an OSCA ID (for senior citizens), please do not forget to include your ID number after your date of birth.
Please make sure to enter all the details correctly especially your email address for all documents will be automatically sent there after the consult. Kindly double-check your contact number as well and always keep your lines open, in case we need to call or message you prior to your appointment.
Once logged in, you may settle the payment either via GCash or PayPal (debit/credit) to confirm the appointment. You will then receive an email notification from us that the booking was successful (Please check your spam and junk folders).
You will receive a unique Zoom link for your video consultation via email. Click it on the scheduled date and time, and your doctor will be waiting on the other end.

Choose a private place to attend your online consultation. Make sure to have a stable internet connection and a fully charged device. Also, download the Zoom app.
If available, we encourage you to prepare some tools such as a BP apparatus, thermometer, and pulse oximeter, to be used to take your vital signs during the consultation. This will be very helpful to our doctors to ensure proper and accurate assessment of your condition.
For re-scheduling, we do allow re-scheduling of appointments but we do require that you advise us at least 1 hour before your appointment. Failure to do so would result to a missed appointment and would forfeit your payment.
For cancellations, as we follow a NO REFUND POLICY, no refund can be given, but a consult can be credited to your account instead that you can use in the future. This credit consult is transferrable and can also be used by one of your peers or family members.

No, we don’t have a pediatrician in the clinic at the moment, however, our general physicians can be able to provide initial pedia consults. We wish to provide you your main point of care and assess you as comprehensively as possible. Any referrals to specialists like a pediatrician may be determined from there on.
Our general physicians can provide initial pedia consults; however, we don’t have a pediatrician. We are still on the process of expanding our roster of specialist doctors in our clinic.
Our doctors can issue medical certificates based on their discretion aligned with their assessment of your condition during the consultation. Since Eva is a telehealth service and there is limited physical examination, please note that we specify “may resume/return to work” rather than “fit to work” on the certificate.
In addition, we don’t issue pre-employment medical certificates and other documents that warrant a physical exam, and back-dated health certificates. There may also be an additional fee for medical certificates issued by specialists
Yes, we have a Urologist in the clinic. We are advocates for men’s health and we are dedicated to provide you the best care.

Your prescription will be sent to you by your physician immediately after your consultation via email. If you’re not able to find your prescription in your inbox, please check your spam or junk folder.
No. That’s the good thing about EVA. You don’t have to leave the house or office to see a doctor. All consultations are held online.
As per the DOH-NPC joint memorandum circular 2020-0001 on the use of telemedicine, the scope of the doctors’ services is limited to patients who are physically residing in the Philippines. In addition to this, we cannot guarantee that the medical documents (such as medical certificate, prescriptions, and etc.) will be valid or honored in another country. We do hope to serve you once you are back in the Philippines!

If you have been previously seen by our doctors regarding your COVID infection, they can provide you with the clearance certificate upon follow-up and once you have completed the recommended isolation/quarantine duration.
If it would be your first consultation with us regarding COVID and would need a clearance from the infection, our doctors would require you to submit a certificate issued by your local government unit as proof that you have completed the recommended isolation/quarantine duration. Without this document, our doctors cannot issue the clearance.
Signing of other documents not issued by EVA depends on its content. Kindly send us the form you would like to be signed BEFORE booking an appointment so our doctors can evaluate it first. These are always accomplished upon the doctor’s discretion.
